Hospital Liaison

When it’s time to come home from the hospital or rehabilitation, CVAA is there to guide the road to recovery. We seek a part-time (32 hours/week), benefit-eligible, Registered Nurse to work with area hospitals, nursing homes, and our team to help transition individuals safely from hospitals and care facilities. 

Our ideal candidate will be an enthusiastic voice for our mission and have familiarity with developing a continuum of care, including performing wellness assessments, communicating with providers, assisting with medication reconciliation (HomeMeds), and developing an action plan for clients. Qualified candidates will have experience working within clinical and case management care settings, preferably with seniors. Registered Nursing degree preferred. This position will work with two of the three hospitals in our region. Location: Chittenden, Addison, Grand Isle & Franklin counties.

Specialized Case Manager

The aging population is often faced with complex needs, requiring the support of an experienced and knowledgeable Specialized Case Manager. This individual works with at-risk clients, their caregivers, and health care providers to deliver person-centered care coordination and ensure the needs of the client are being met.

CVAA’s ideal candidate will have a background working with people who may have cognitive challenges, chronic medical conditions, and/or mental illness. Qualified candidates must possess a minimum of a bachelor’s degree (BA/BS) and three years of experience working in a community health setting, preferably with seniors, people who have disabilities or other human service populations. MSW or related degree preferred. Location: Essex Junction, VT.

Internship Opportunities

CVAA is pleased to offer internship opportunities for people who want to explore the social services field, obtain experience working with seniors, develop career skills, and meet internship or field experience requirements.  We make every effort to tailor our internships to the needs of CVAA and the interests of the intern.  All internships are unpaid and have a flexible weekday schedule.
