Choices for Care
You have the right to choose how to live your life.
CVAA Case Managers are person-centered. We take our direction from you, because we believe you know what you need better than anyone else.
Choices for Care is a long-term care program, funded by Medicaid, to pay for care and support for older Vermonters and people with physical disabilities. The program assists people with everyday activities whether they are at home, an enhanced residential care setting, or a nursing facility.
The program provides support that allows participants to maintain their independence. These include hands-on assistance with eating, bathing, toilet use, dressing, and transferring from bed to chair; and assistance with tasks such as meal preparation, household chores, and medication management.
What you can expect from your CVAA Case Manager in the Choices for Care Program:
- help to understand the eligibility criteria and options available to your through the Choices for Care program
- assistance to you and your family in navigating the application process for Long Term Care Medicaid to help finanance your participation in Choices for Care
- assistance obtaining durable medical equipment or home modifications to keep you safe and comfortable in your home
- advocacy and coordination of services with other agencies and providers
- a monthly visit in your home to monitor services and assess your strengths, needs and eligibility for additional benefits and services
Participants in Choices for Care must qualify for Level II nursing home placement and meet financial eligibility criteria. They have a choice of service providers and services under the program.
